Birth/Death Certificate Request:
For already registered births/deaths, they can request for birth certificate in any of the eSeva Centers in twinscities or eSeva website.
Walk into any eSeva center and fill in the application form available at eseva for certificate request. Based on your details eseva operator will verify the data whether the birth/death is already registered?
If registered, pay the required fee amount of Rs.20/ per certificate and eSeva service charges Rs.15/- and required courier charges as per the option below
Applicant can choose the option for delivery of certificate
1) At eSeva Rs.15/-
2) Within GHMC Limits Rs.25/-
3) With in AP (out side of GHMC) Rs.40/-
4) With in India Rs.60/-
If the applicant chooses the option 1, applicant has to come to eSeva for collecting the certificate/s after 3 working days(excluding the date of application).
If option 2 or 3 the certificate/s will be courier to applicant address after receiving the certificate/s from the GHMC Circle offices.
Inclusion of the Child Name in the Birth Register:
For already registered births for which child name is not included, they can request for inclusion of child name in the birth register in any eSeva.
Walk into any eSeva center and fill in the application form available at eseva for inclusion of child name in the birth register.
Based on your details eseva operator will verify the data whether the birth is already registered and child name is not updated
The applicant should be child, father or mother only.
Within One Year of Birth Date:
The following documents are required along with the filled application form.
Both the father and mother should give a declaration that, they have named the child as __________ and both of them should sign on the paper.

Ration Card/any other residence address proof and School bonafide certificate. If both are not available, produce notarized affidavit on Rs.10/- non-judicial stamp paper
If registered, pay the required fee amount of Rs.20/- per certificate and Rs.10/- late fee for inclusion of name after one year of birth date. eSeva service charges Rs.15/- and required courier charges as per the option below

For Delivery Type other than 1,The certificate/s will be courier to applicant address after receiving the certificate/s from the GHMC Circle offices.
Corrections in Birth/Death Certificates:
Walk into any eSeva center and fill in the application form available at eseva for corrections in birth/death certificate.
Based on your details eseva operator will verify the data whether the birth/death data is already registered?
The following documents are required along with the filled application form.
Corrections in Birth Register:
1. All the original certificates of birth already taken should to be returned.
2.Declaration By the parent(s) attested by two Gazetted officers.(Name and Designation of the Gazetted Officers are to
be written in Capital)
3. Notary Affidavit on (Rs. 10/- Non-Judicial Stamped Paper).
4. Documentary evidences like
a) Education Certificates
b) Election ID Card
c) Ration Card
d) Passport or any other registered documents.Documents like Pan Card, LIC and Bank can not be taken up
5. Other child certificate if any
6. Consent letter from the concerned Hospital regarding the correction to the effect.
7. Any other documents if any
Corrections in Death Register:
1. All the original certificates of death already taken should to be returned.
2.Declaration By the parent(s) attested by two Gazetted officers.(Name and Designation of the Gazetted Officers are to
be written in Capital)
3. Notary Affidavit on (Rs. 10/- Non-Judicial Stamped Paper).
4. Documentary evidences like.
a) Education Certificates.
b) Election ID Card.
c) Ration Card.
d) Passport or any other registered documents.Documents like Pan Card, LIC and Bank can not be taken up.
5. Letter from the hospital authorities where death has occurred mentioning the facts.
6. In case of Medico-legal cases
a) FIR
b) Post Mortem Report
c) Education Certificates.
d) Election ID Card.
e) Ration Card.
f) Passport or any other registered documents.Documents like Pan Card, LIC and Bank can not be taken up.
7. Any other documents if any
If application fulfill all the above, pay eSeva service charges Rs.15/- and required courier charges for delivery of certificate at eSeva Rs.15/-.
Certificate will be delivery to only at eSeva for Corrections of birth/death events.Applicant has come to eSeva for collecting the certificate/s after 10 working days (excluding the date of application).
If Correction Approved , The required fee amount of Rs.20/- per certificate and Rs.60/- for extra for corrections in birth/death registers towards GHMC fee. The Fee Amount will be collected at the time of issuance of correction order.
If Correction Not Approved By The Registrar the application will be returned to the applicant
Note:
Complete change in name cannot be carried out as per the Births& Deaths Act,1969. Only spelling mistakes, expansion of the initials, prefix and suffix to the existing name can only be carried out subject to the satisfaction of the REGISTRAR.
Non-availability Certificates of Birth/Death:
Walk into any eSeva center and fill in the application form available at eseva for Non-availability Certificates of birth/death. Based on your details eseva operator will verify the data whether the birth/death data is already registered?
If the record not found, applicant has to apply for non-availability certificates.
The following documents are required along with the filled application form.
Non-availability Certificates of Birth :
1. Ration card/any other residence proof.
2. School bonafied certificate.
3. Secondary school marks sheet.
4. Notarized affidavit on Rs 10/- Non-Judicial Stamped paper.
5. Any other documents if any.
Non-availability Certificates of Death:
1. Ration card/any other residence proof.
2. Notarized affidavit on Rs 10/- Non-Judicial Stamped paper.
3. In case of Medico-legal cases.
a) FIR
b) Post Mortem Report
c) Form-2 given By Concerned Police Authorities
4. Any other documents if any.
If application fulfill all the above, pay eSeva service charges Rs.15/- and required courier charges for delivery of certificate at eSeva Rs.15/-.
The Non Availability Certificate of Birth/Death will be delivered to only at eSeva.
Applicant has come to eSeva for collecting the Non Availablity Certificate/s of Birth/Death after 20 working days (excluding the date of application).
How can I receive registered death certificate ?
If application fulfill all the above, pay eSeva service charges Rs.15/- and required courier charges for delivery of certificate at eSeva Rs.15/-.
I submitted an application for inclusion of child name at Mee sSeva. After three months I applied birth certificate at Mee Seva. In that form at the name column, name was not showing. Then I apply to inclusion of child name. It shows as name was included. So how can I get the birth certificate with name.
