How to Apply For Pcpndt License :
Steps for Filling the online E-Registration Form :
1.Name of the Proprietor-Enter Hospital Proprietor Name
2.Name of the Organization-Enter Full Name of the Hospital to be registered.
3.Email-id-Enter a vaild Email-id (preferably general or the hospital proprietor email id)this will be used for further Communication and Updates.
4.Mobile No-Enter the Valid Mobile No For Registration updates and further Communication.
5.Type of Institution-Select Type of Hospital from the drop down.If any Other Type then select any other from drop down and Enter the type in the text box.
6.Type of Ownership-Select Type of Ownership from the drop down.If any Other Type then select any other from drop down and Enter the type in the text box.
7.Full Name and address-Enter the Hospitals Address which will be displayed in the PCPNDT FORM-B CERTIFICATE of REGISTRATION.
8.Type of facility to be registered-Kindly click on the appropriate check box to select the Type of facility to be registered and which is present in your hospital
9.Type of Hospital-Select the appropriate option here, if your Hospital has In Patient Facility or Not.
10.Specific Prenatal Diagnositic-If Invasive Technique is used in your hospital then check the corresponding procedures done
11.Non Invasive Ultrasound-Please tick this option if your establishment conducts Non invasive ultrasonography.
12.Facilities available in counselling centre-Kindly tick the facilities available in your Genetic Counselling Center.
13.Whether facilities are or would be available in the Laboratory/Clinic for the following tests-Select the tests made available for the patients in your Laboratory/Clinic.
14.Whether facilities are available in the Laboratory/Clinic for the following-Select the appropriate facilities.
15 State whether the hospital/Clinic is eligible to be registered as per rule 3 of PC and PNDT Rules 1996-Select yes or no. If no then your hospital is not eligible for license registration.
16.Enter the details of doctor conducting the scans in your establishment-Enter all the details and click on add button to add more doctors. To Remove a doctor select the added record and click on remove
17.Enter the Details of all the Ultrasound machines Available in your Establishment-Enter all the machine details and click on add, to remove select the record and click on remove.
18.Upload Documents-You will be able to upload only PDF documents, so kindly scan all the documents required in the PDF format and the size of each document should be less then 1Mb.

Steps to upload Documents :
18.1. Click on upload document button a pop up will appear in order to upload documents.
18.2. There are 7 Documents Need to be uploaded and all should be in PDF format and lest then 1 MB in size.
18.3. Documents That Needed to be Uploaded are as follows
18.3.1 AFFIDAVIT ( Rs.50/- STAMP PAPER, Notarized by the NOTARY )
18.3.2 Scanning Doctor Qualification Certificates(kmc And Ultrasonography Training Certificates In Case Of Mbbs Doctors)
18.3.3 Owner Qualification Certificates
18.3.4 Scanning Room Sketch
18.3.5 Scanning Machine Invoice Copy
18.3.6 Declaration
18.3.7 Requisition Letter To Issue New Pcpndt License Copy of KPME Certificate/copy of Acknowledgement for submission of the same.
18.4. Check the type of document to be uploaded then click on browse and select the document do this for all 7 documents.
18.5. Ensure documents represent same thing against uploaded document type.
18.6. Click on upload button and wait until uploading is finished successfully then the close the pop up.

19.Select payment Type :
Select the payment mode.Currently we support only DD and NEFT/RTGS
For Hospital with Multiple Facilities,the Registration Amount is Rs.35,000 and for a hospital with Single Facility Registation Amount is Rs.25,000
If your payment type is Demand Draft, you will have to purchase the D.D in favour of “Appropriate Authority, D. H. & F. W. O. BANGALORE”
and then submit the same with your application no and the email acknowledgement received to the DHO office, with in Seven Days. If your payment type selected is RTGS/NEFT,plz transfer the amount to the following account no

Kindly scan and send us the Voucher along with your application no to process your application.

20.Submit- Click on submit button on successful submission you will get an application no which will be used for further application tracking.

How to Update License Information :
Steps for Updating online E-Registration Form :
1.If You Get an EMail with regards to Rejection of your application the some of the information provided by you might Not be Correct or some Document you have uploaded is not as per the requirement asked in the PC and PNDT ACT, so kindly go to the License Update Tab and update the information asked
2.Enter Your Application No and click on submit all the data will be displayed.
3.Enter the data that needs to be corrected .If some Document is wrong
4.Click on Upload Document it will display document type which needs to be uploaded select and upload.
5.Click on submit, your application will be updated and message will be sent to you for successful update.
