Instructions For Advertisers :
1 : In order to ensure correctness, notices of change of name should either be typewritten or written in BLOCK LETTERS, on the term provided. No correction is allowed after the term is accepted. The Notice will be accepted only in the proper form provided and not in any other form.

2 : illiterate persons should impress their left hand thumb impression on the line reserved for this purpose. The name of the person should be below the thumb impression for which separate space is provided.

3 : In case of minors (below 18 years), this notice form should be tonuarded through a legal practitioner or should be countersigned by the guardian of the applicant. Students studying in Schools/Colleges should submit their forms to this office through the Heads of their Schools/ Colleges.

4 : The advertisement charges are strictly payable in advance. They cannot be realised by V.P.P. Two copies of the Gazette will be supplied in the normal charges paid for the advertisement of the change of name etc.

5 : The revised rate for Change of name in the Maharashtra Government Gazette, is fixed at Rs. 129 (Rupees One Hundred Twenty only} her insertion, inclusive of Two Copies of the Gazette {to be supplied free). For Backward Class applicants a rate of Rs. 60 (Rupees Sixty only) is fixed. The charge per extra copy of the Gazette will be Rs. 15 (Rupees Fifteen only). if extra copies are required the cost thereof may be remitted within 7 days from date of receipt of Gazette as iimited copies are printed. Backward /’ Scheduled Caste applicants, may send a attested xerox copyof caste certificate of old name issued by the Coliector of the District concerned /Tahsildar.

(2)Vimukta Jatis,
(3) Nomadic Tribes,
(4) Nav Buddhas,
(5) Tribals outside the specified areas of Vidarbha and
(6) Scheduled Castes,
(7) Special Other Backward Class are included in Backward mass].

Other Backward Classes are not considered forconcession. Ration Card/or Light Biii zerox copy attached for Name & Address Proof.

6 : The publication in the Maharashtra Government Gazette or a notification regarding change of name confers no right on the person concerned to have the name changed on Government or other records. Similarly, Government does not take any responsibility. if the notice of change of name is given by any unauthorised person, since publication of such notice is an advertisement and not an authentic record or proof for the name so changed. One form may be used for one person only.

7 : Necessary advertisement charges as per scheduled rates may be remitted by Bank Draft! or by Cash on counter in the office of the Directorate, Government Printing, Stationery and Publications, Netaji Subhash Road, Charni Road, Mumbai 400 004. However forms duly filled in may be sent well in advance. Cheques will not be accepted.

8 : Printing mistake, if any. published in the Maharashtra Government Gazette should be reported to the Director, Government Printing. Stationery and Pubiications. Netaii Subhash Road, Charni Road, Mumbai 400 004 within seven days from the ~ date of receipt. No cognizance will be taken thereof thereafter.

9 : It should please be noted that normally a THREE months’ time is required for publication of the advertisement, in the Maharashtra Government Gazette from receipt of full cash thereof. Gazette copies are sent by Post Under Certificate of Posting. if the Gazette is not received within this period, a complaint should be preferred within seven days with all details. Complaints :nade thereafter cannot invariably be entertained into.

10 : Registration number and old Name may be quoted while corresponding with this office.

11 : No advertisement is accepted for change of castes or sub-castes.

12 : No advertisement is accepted for Change of Name of dead persons
