Sri Devaraj Urs Academy of Higher Education and Research, Tamaka, Kolar, Karnataka will be conducting an All India Entrance Test for admissions to Under Graduate Medical courses for the Academic year 2014-15 on 20.04.2014 (10.00AM to 1.00PM)

ELIGIBILITY CRITERIA FOR ADMISSION TO MBBS:
Eligibility criteria for admission to MBBS course is strictly based on Rules and Regulations of Medical Council of India.
The candidate must have passed the qualifying examination as under:
The higher secondary examination of the Indian school Certificate Examination which is equivalent to 10+2 Higher Secondary Examination after a period of 12 years study, the last two years of study comprising of Physics, Chemistry, Biology and Mathematics or any other elective subjects with English at a level not less than core course of English as prescribed by the National Council of Educational Research and Training after the introduction of the 10+2+3 years educational structure as recommended by the National Committee on education OR other qualifications as recognized by Medical Council of India.
The Candidate should have passed the qualifying examination which is 2nd PUC or 10+2 higher secondary or equivalent examination. The last two years of study comprising Physics, Chemistry and Biology (PCB) with English as a compulsory subject, must have passed these subjects individually and must have obtained a minimum of 50% of marks taken together in Physics, Chemistry and Biology. In addition, in the competitive entrance test, the candidate must have secured not less than 50% marks in Physics, Chemistry and Biology.
Age: The candidates should have completed 17 years of age on or before 31st day of December of the year of admission.
General Instructions to Candidates :
Please ensure that you have the following ready at hand before you start filling up the Online Application Form
Note: Send the Application in A4 size envelopes only. Do not fold the application form
Super scribe the envelope at the top “SDUAHER-AIUGMET-2014 Application Form”
Demand Draft drawn in favour of The Registrar, SDUAHER payable at Kolar for Rs.1500/- (One Thousand Five Hundred only).
Ensure that the online application form is filled up properly and completely.
After you have filled your Online application form, click on the submit button only once to send the details to the server. The server will assign you an application number, and provide the facility to view/print your online application form with the details you have filled, along with the application.
Take printouts of
1. The filled in Application form
2. Date of Birth Certificate / Identity Proof Candidate’s Copy
3. Date of Birth Certificate / Identity proof SDUAHER Copy.
The completed application form must have the signature and Forefinger impression of the left hand of the candidate.
Do not send any original documents along with the application form.
Kindly paste your recent (not older than three months) passport size colour photograph, only in the space provided. Application with smudged / tampered or hazy photograph will not be accepted. Do not staple or pin your photograph to the application form. Do not Sign on photograph. No attestation is required on the photographs.
Please adhere to the time schedule enlisted in the calendar of events.
Incomplete / incorrect applications are liable to be rejected without prior information to the candidate.
Any application received after the last date is liable to be rejected.
Attested photocopies of –
1. SSLC or 10th Marks card (Compulsorily).
2. 12th or 2nd PU Marks card (If available).
3. SC/ST/OBC certificate and Income certificate issued by competent authority not less than the Rank of Tahsildhar.
Additional photograph : 6 copies of color photographs identical and clear in all respect (mention your application number on the back of the photographs)
Please send the following along with the completed Application Form:
DD drawn in favour of “The Registrar, SDUAHER, Kolar”, payable at Kolar for Rs.1500/-. Kindly ensure banker’s Authorized Signatories on the Demand Draft. Mention your application number on the back of Demand Draft.
SDUAHER Copy of the Date of Birth Certificate / Identity Proof (Once you submit your online form, you will also have a provision to print a format of this form, which must be attested by the Head of the Institution where he/she last studied or gazetted officer from the place of residence of the candidate). Keep the candidate copy of the Date of Birth Certificate / Identity Proof with you. Attach the SDUAHER Copy of Date of Birth/Identity Proof along with the Application Form.
Additional photograph: 6 copies of your recent passport-size sharp and clear colour photograph with the application number written on the back of the photographs.
Attested photocopies of –
1. SSLC or 10th Marks card (Compulsorily).
2. 12th or 2nd PU Marks card (If available).
3. SC/ST/OBC certificate and Income certificate issued by competent authority not less than the Rank of Tahsildhar.
]The application can be submitted by post or in person (acknowledgement has to be obtained if submitted in person). The application form complete in all respect has to be Addressed to : The Registrar,U.G. Admission Cell , Sri Devaraj Urs Academy of Higher Education & Research, Tamaka, Kolar – 563101, Karnataka, India to reach on or before 09.04.2014, 5.00 PM.
