** Demand Draft of Rs .120/- (Rs. One hundred and twenty Only) in favour of Registrar Maharashtra Medical Council payable at Mumbai. Or Cash.

Additional Qualification Registration :
Instructions :
** Produce following Original certificates for verification along with their attested photocopies and Prescribed Application Form :
** Passing Certificate / P. G. Degree / Diploma of additional qualification issued by university.
** Bonafide Certificate issued by the Head of Institute / Head of Department.
** M.M.C. Registration Certificate & copy of I-Card issued by MMC/Confirmation letter of MMC Renewal.
** Demand Draft of Rs .120/- (Rs. One hundred and twenty Only) in favour of Registrar Maharashtra Medical Council payable at Mumbai. Or Cash.
** If you have changed your name please attached a zerox copy of M.M.C. letter.
** Two Copies of latest photographs of passport size.
** Recongnisation list of Other State qualification from Medical Council of India’ website

Change of Name :
Instructions :
** Produce Original and attested photocopies of Certificates
** In Case of Change Of Name
** Application for Change of Name.
** Original Certificate of Maharashtra Medical Council Registration and Identity Card.
** Marriage Certificate i) Govt. gazette ii) affidavit (Any one).
** Three copy of Latest passport size Photograph.
** Payment Rs. 70/- for Change of Name and separate Demand Draft for Rs.120/- for Duplicate Certificate. Demand Draft in favour of Registrar Maharashtra Medical Council payable at Mumbai. Or Cash

In Case of Change Of Name Male :
** Application for Change of Name.
** Original Certificate of Maharashtra Medical Council Registration.
** No Objection from Dean of Parent College certifying he is same Bonafide person Govt. gazette
** Affidavit with photograph.
** Three copies of Latest passport size Photograph.
** Payment Rs. 70/- for Change of Name and separate Demand Draft for Rs.120/- for Duplicate Certificate. Demand Draft in favour of Registrar Maharashtra Medical Council payable at Mumbai. Or Cash

Issue of No Objection Certificate (N.O.C.) :
The Maharashtra Medical Council, issues No Objection Certificate on demand in respect of the R.M.P.s, who desire to register themselves with other State Medical Councils.
