Documents required to be submitted along with Application Form:
Domicile Certificate :
Application form duly attested by a Class – I officer that he/she knows the applicant for more that 3 years. The above attestation should be done only on the basis of personal knowledge of atleast three years by any Under Secretary of Delhi Govt/ Govt of India, any Gazetted Officer holding Class-I post of Delhi Govt/ Govt of India, any Magistrate of Superintendent of Police. In case attesting officer makes wrong attestation either willfully or due to insufficient information in the application will be liable to criminal proceedings as well as departmental action as deemed fit.
** Voter I-Card/ Ration Card – As proof of present residence
** Proof of continuous stay in Delhi for last 3 years, such as Education certificate, Electricity Bill, House Tax, Water Bill etc.
** Proof of Date of Birth. (Date of Birth Certificate issued by MCD of 10th Class Certificate)

Schedule Caste/ Scheduled Tribe Certificate :
** Attestation by Gazetted Officer, Member of Parliament, MLA, Councilors and Member of SC/ST/OBC Welfare Board declaring the caste of Applicant.
** Copy of SC/ST certificate of Father, Brother, Sister of his/her blood relative from parental side, if available.
** No Affidavit is required to be furnished by the applicant who append copy of SC Certificate of their father side issued from Delhi state. However, in case of applicants who applied on the basis of SC Certificates issued by other states the affidavit is required..
** Two passport size photographs, one attested and one plain.
** Residence proof (Voter I-Card/ Ration Card/ Passport/ Electricity Bill/ Water Bill/ Telephone Bill etc.).
** Birth Certificate of School Certificate.
** Where no SC/ST Certificate has been issued to any member of applicant- s family, two witness who are Government servants are required who should give in writing that they know the person and his caste, along with the attested copies of their identity cards.
** In case of a married woman, the local enquiry is conducted at the place of residence prior to the marriage as well as the place of residence after marriage.

Other Backward Classes Certificate :
** Attestation by Gazetted Officer, Member of Parliament, MLAs, Councilors and Member of SC/ST/OBC Welfare Board declaring the caste of Applicant.
** Copy of OBC Certificate of Father, Brother, Sister of his/her blood relative from parental side, if available.
** No Affidavit is required to be furnished by the applicant who append copy of SC Certificate of their father side issued from Delhi state. However, in case of applicants who applied on the basis of SC Certificate issued by other states the affidavit is required.
** Two passport size photographs, one attested and one plain
** Proof of residence in Delhi since 1993 such as Electricity Bill, Water Bill, Telephone Bill, House Tax etc.
** Affidavit showing the caste, income and agricultural land owned by the applicant, if any
** Birth Certificate or School Certificate

In case of married woman :
1. Affidavit from her father showing his income and caste
2. Copy of OBC Certificate of the Father/brother, if any

Income Certificate :
** Application form completed in all respect
** Affidavit in prescribed format on a Rs 10 stamp paper
** Voter I-Card/ Ration Card/ Passport-As proof of present residence
** Proof of continuous stay in Delhi for last 3 years, such as Education certificate, Electricity Bill, House Tax, Water Bill etc
** Income Proof (Salary Certificate from company, in case of hawkers etc electricity bill, telephone bill etc).

Handicapped Certificate :
Application form in prescribed format.
Voter-I-Card/Ration card-As proof of present residence.
Medical certificate of Delhi approved Hospitals (LNJP/RML/Safdarjung/GTB/Hindu Rao/AIIMS/Military Hospital).
Two photographs, one attested one plain.

Birth Certificate :
** Application form in prescribed format
** Affidavit in prescribed format on Rs. 10 stamp Paper
** Voter I-Card/ Ration Card-As proof of present residence
** In case of Death Certificate-Cremation Slip/ proof of burial

Death Certificate :
** Application form in prescribed format.
** Affidavit in prescribed format on Rs. 10 stamp Paper.
** Voter I-Card/ Ration Card-As proof of present residence.
** In case of Death Certificate-Cremation Slip/ proof of burial

Surviving Members Certificate :
** Application Form on plain paper.
** Affidavit of all Surviving members attested by notary.
** Death Certificate.

Affidavit on the following effects :.
a. Details of deceased person i.e name, age, address
b. Details of surviving members, their ages and relationship with the deceased
c. Purpose for which the certificates is required

** Voter I-Card/ Ration card/ passport or any other proof – as proof of present residence
** Copy of letter/ reference of the authority before whom the surviving members certificate is required (if any)

Nationality and Citizenship Certificate :
** Application form duly attested by Class-I Gazetted Officer of Delhi Govt/ Govt of India.
** Voter I-Card/ Ration Card/ Passport or any other proof – as proof of present residence.
** Proof of continuous stay in Delhi for last 3 years, such as Education certificate, Electricity Bill, House Tax, Water Bill etc.
** Photocopy of Birth Certificate duly attested.

Solvency Certificate :
** Application form in prescribed format.
** VVoter-I-Card/Ration card – As proof of present residence.
** Proof of profession like Pay certificate, Income Tax Return etc.
** Details of immovable property alongwith documents.
** Valuation report of Govt. approved valuer
** Proof of rented value, if any
** Proof of the property being free from all encumbrances, if any
** Proof of House Tax payment, if any
** In case of agricultural land, its size and details of location with Khasra No.
** Name of the owner/co-owners with share of each
** Copy of latest Revenue record like Fard
** Details of Bank Account
** Details of Financial Assets like Bonds, Share Certificate, Fixed Deposits, Provident fund etc.
** In case of Company, details of Financial structures
** Income Tax returns for last three years
** Share Holding of all Directors & their financial status
** Affidavit in the prescribed performa
