West Bengal Health For All Employees and Pensioners Cashless Medical Treatment Scheme 2014 Online Application :
Step 1: The Applicant will access http://wbfin.nic.in and will click on ‘Health Scheme Portal of Finance Department’

Step 2: In the Home Page of ‘Health Scheme portal’, there is a button “Online Registration”. Through clicking the button, the User will get “Procedure and guidelines for online Application under ‘West Bengal Health for All Employees and Pensioners Cashless Medical Treatment Scheme 2014’”with options for selection for “Employee” or “Pensioner / Family Pensioner”

Step 3: Based on selection , the “Online Registration for Employee” or “ Online Registration for Pensioner / Family Pensioner” will be available

Step 4: For “Online Registration for Employee”, the Employee has to register with Login Id as GPF Account No and Date of Birth as password. For “Online Registration for Pensioner / Family Pensioner”, the Pensioner / Family Pensioner has to register with Login Id as PPO No. and Date of Birth as password.

Step 5: After successful login, Data Entry Screen will be available for Employee / Pensioner / Family Pensioner with following buttons.

For Employee:
a) Personal Details
b) Family Details
c) Present Office Details
d) Respective CCA Details
e) Respective DDO details

For Pensioner/Family Pensioner:
a) Personal Details
b) Family Details
c) Last Posting Office details
c) Last Posting Office details
d) Respective Pension Sanctioning Authority details

The Applicant has to enter Mobile No. and Voter ID. Card No./PAN No. mandatorily. The Applicant must know his/her DDO Code.

Step 6: After entry of above said details and clicking the ‘Submit’ button, Online Registration of the Employee / Pensioner will be completed and one Application ID will be generated. A ‘Print’ button will be available for ‘Printing the Application Form”.

Step 7: Employee and Pensioner / Family Pensioner will take three copies of print out of the Application Form. A stamp size recent colour photograph of each of the beneficiary will have to be pasted at the earmarked place on all the three copies of the application. The beneficiaries will also be required to put their respective signature at the earmarked place on all the three copies of the application. In case of a child below twelve years of age, natural or legal guardian will put signature on behalf of the ward.

Step 8:The Employee has to submit those Printed Application Forms along with one additional stamp size photograph of each beneficiary to the respective DDOs. DDOs will return one copy to the Employee concerned after affixing his signature and seal as an acknowledgement of receipt and retain the second copy in the respective office for record. The third copy of the Application duly authenticated by the DDO will have to be sent to the Medical Cell, Finance Department, Writers’ Buildings, Kolkata – 700001.

Step 9: The Pensioner / Family Pensioner has to submit the above said Printed Application Forms along with one additional stamp size photograph of each beneficiary to the respective Pension Sanctioning Authorities (PSAs). PSAs will return one copy to the Pensioner / Family Pensioner concerned after affixing his signature and seal as an acknowledgement of receipt and retain the second copy in the respective office for record. The third copy of the Application duly authenticated by the PSA will have to be sent to the Medical Cell, Finance Department, Writers’ Buildings, Kolkata – 700001.

  • I am a retired officer of the GOVT OF WEST BENGAL. I like to come under WBHS 2014.The beneficieries will be self ,wife and her mother if allowed. But my wife Narayani Gupta has a call name "JABA" And her PAN CARD,VOTER ID CARD etc. Her name has been recorded as "JABA GUPTA.' whereas in the PPO her real name has been recorded. In the Pension paying joint A/C.her name is NARAYANI GUPTA. For this purpose court affidavit and publishing in newspaper have been done. Will this make any inconvenience? Or whether the copy of affidavit and newspaper cutting have to be sent? I retired ON 31st August 2003. Whether I am eligible to apply?Kindly instruct.Thanking you JAYDEV GUPTA.

  • I am a retired Govt. employee. I have been entered in the WBHS 2008 in 2012. My wife is a retired primary school teacher, but at that time she was not been included due to unknown reason from my Office (O/F the Exe.Engineer,Pwd ,Purta Bhaban,Div.I ,Saltlake). I have again applied to the office to include my wife's name near about 1 year's ago and after that I have submitted all necessary documents of my wife as per the instruction. But no improvement has been seen by me. So, Please instruct/guide me about my further steps.
