Required Supporting Documents :
1. Certified Copy of electoral roll or serial no of the applicant in the voter’s list
2. Birth Certificate of all family members mentioned whose names are not included in the Voters list
3. Land Revenue(Khazana) Receipt of the applicant / applicant’s father
4. If the applicant is staying on rent, then the land revenue(Khazana) receipt of the house owner
5. If the applicant possessed any other FIC obtained at some other state/district, then Surrender certificate of the FIC issued earlier has to be submitted
6. If the applicant is not staying with his father, then application from the father seeking separation of the name of the applicant(son) from the original FIC,Linkage to be established for issuing FIC in the applicant’s name
7. If applicant staying separately from his father, then Goanburah’s certificate stating the separation (Certificate of Seperation from GP for rural areas and from Municipal Board for town ares) Municipal Receipt having holding no of the house
8. Documents (if any)
9. If the applicant came on transferred from any GPSS area of the district he must furnish the surrender certificate duly countersigned by area inspector along with the cancelled F.I.Card
10. P/photo of the applicant

Service Time Frames :
I. With Dealing Assistant :
In Days : 1
Description :
** Case to be handled within 1 day after the receipt of application.
** Dealing Assistant should take action before the end of 2nd working day from date of application.

II. With Approving Authority :
In Days : 6
Description :
** Approving Authority to review the Application and take appropriate action (Approve / Reject / Verify) within 6 days from the date of the application.

III. Days for Final Action :
In Days : 7
Description :
** The Service is to be catered to within 7 working days from the date of receipt of the application.
