Application Form of Request for Residence/Nativity/Nationality Certificate :
Applicant can fill an application e-form for request of Residence/Nativity/Nationality Certificate.

How to apply :
The applicant who wants to apply for request of Residence/Nativity/Nationality Certificate shall submit the e-form duly filled-in. He/she shall state true facts in the application. Follow below instructions to access the application,
1. Click on Services and Forms on the portal.
2. Select Revenue Department.
3. List of all services under the department will be displayed. Depending on the method of submission, click one of the links displayed under Action.
4. Acknowledgement with a unique acknowledgment number will be provided.
5. Using the unique registration number, the applicant can check the status of the application on the state portal.
6. Once the status is complete the user has to meet the concerned department person along with the required original documents and collect the certificate from the department person.

The required documents mentioned below are required to be submitted along with the e-form.

Maximum file size should not be more than 2MB.

Attach scanned copy of :
1. Birth Certificate.
2. Ration card.
3. Electoral Photo Identity Card (EPIC).
4. School records such as Transfer Certificate etc.
5. Employment card / Service Certificate from the Employer.
6. Previous Certificates of the applicant/ Blood relatives, if any.
7. Passport.
8. Other proofs.
9. Applicant’s photograph

Applicant can fill an application e-form for request of income certificate.

How to apply :
The applicant who wants to apply for request of income certificate shall submit the e-form duly filled-in. He/she shall state true facts in the application. Follow below instructions to access the application,
