Govt of Maharashtra ID Card for senior citizens :
In the developed countries of the West and East senior citizens are treated with care and concern giving cognizance to their special life circumstances. Almost all western countries issue Social Security ID cared that entitles the card holder to avail of benefits at shops, service centres and products exclusively meant for senior citizens .

In 2001 Dignity Foundation was able to convince the state government, appealing to the Chief Minister, that senior citizens require a special identity card, in order to distinguish their segment for many schemes of assistance.

Through a GR dated Sept 24, 2001 and several other successive GRs Dignity Foundation was empowered to issue the ID card with certain guidelines issued for documentation and verification of data provided by the senior citizen.

For the first time in India the all-purpose ID card was inaugurated by the Chief Minister of Maharashtra at a function organised by Dignity Foundation and Rotary Club of Bombay Pier on December 8, 2001 at the Chowpathy sands, Mumbai. Under his instructions, the Ministry of Social Welfare arranged for the issue of ID card against payment of Rs. 30. Subsequently this price was changed to Rs.50.

Administrative charge has been vested in Dignity Foundation and FESCOM – two NGOs who took the initiative to work for the issue of cards.

At present Dignity Foundation has 83 ID Card Centers all over Maharashtra, including 13 centres in Mumbai alone and are in process of opening centres in all the taluka head quarters of Maharashtra. Till 2010 around 9 lakhs ID cards have been issued by Dignity Foundation.

The benefits of having an ID Card are :
** Railway concession – 30% (60 years).
** Maharashtra state transport concession- 50% (65 years).
** Air travel concession – 50% on basic fare only (65 years).
** Some private hospitals discount – 30% (60 years).
** Govt. Hospitals treatment free of cost – (60 years).
** Banks give extra interest – 1/2% (60 years).
** Post office – on investment upto rs.15 lakh gives – 9% interest (60 years).
** High court will give priority in hearing – (65 years).
** Maharashtra professional tax – 100% exemption (65 years).
** M.TNL – 25% discount on monthly rent – (65 years).
** Railway have separate queue for senior citizens.
** Local trains/ best buses have reserved sitting.
** Income tax relief.
** Old age home admission free or at nominal cost.

During medical emergency details furnished in the card will be of immediate help. The Card bears the stamp and signature of the Tehsildar.

Important Instructions :
** Please do no use carbon paper for second and third copy. These are not acceptable in the Tahsildar’s office.
** Submit Two Photocopies of all documents.
** Only 60 + people are eligible for the Senior Citizen Card.
** We try our best to deliver ID Cards within 2 months to the Dignity Centres, but, this depends entirely on the time taken by the various Tahsildars.
** All errors in the ID card will be rectified ONLY if brought to our notice within 5 days of collecting your card. Not later.
** In case of change in telephone no./ address a new form is required to be filled and submitted along with your existing ID Card with all your particulars (note : all documents are required if this is sent to us after 3 months from date of issue ).
** In case ID Card is lost an FIR needs to be lodged and a new form with original copy of FIR along with your particulars need to be submitted.
** Kindly note : All ID cards will be issued to the card holder in person, failing which, an authorization letter signed by the card holder has to be presented for collection of the ID card.
** Collection of ID card : ID Cards must be collected from the same “Dignity ID Card Centre” where you have submitted the application form or have been instructed to.
** If any senior citizen is not satisfied with Dignity volunteer/staff services, you are invited to jot down their names and represent your problems to me or write to my office address.
** Ordinary Enquires only on 022-61381100 Monday to Friday 10.30 am to 1 pm only.

Check List For Id Card :
All original documents have to be brought while submitting the forms for verification

Proof of Age : (Submit ANYONE of these documents)
** Valid passport
** PAN card
** Pension book
** School leaving certificate
** In case of name change for woman submit marriage certificate
** If the applicant does not have any of the above documents, obtain an Age proof certificate from a Government Hospital or Municipal dispensary not private clinic

Proof of Name : (Submit ANYONE of these documents)
** Valid passport
** PAN card
** Pension book or card
** In case of name change for woman submit marriage certificate

Residence proof : (Submit ANYONE of these documents)
** Ration card
** Valid passport
** Election card
** Electricity bill
** Telephone bill

Medical Information :
** Blood group – Submit report from a Pathological laboratory.
** Medical Information – A Doctor’s prescription to indicate Illness/Diseases/medication in use/medication allergic to.

Photographs :
3 stamp size photographs (Attach two photographs on the white form and one on the pink form)

Eligibility :
** Any one who has completed 60 years of age is eligible.
** The applicant should be resident of Maharashtra.
** Original documents should be brought for verification.
** 2 photo copies of all documents submitted as proof should be shown..
** Forms (in triplicate) can be purchased at Rs.50/-.
** Filled form should be submitted to any of the centres listed.
** It will take around approx. 60 days to 90 days from the day of submission of application for getting ID.
** All centres are open only Monday to Friday, and closed on bank holidays.
** Form for Mumbai is different from Forms of other parts of state.
