How to Fill Pension Claim Form?
General :
1. Always mention your mobile no. in the claim form so that as and when it is settled, you will be intimated about it.
2. Always submit a copy of first page of your bank passbook which should be updated one and should contain the core banking/CBS account number/IFSC code. Nowadays almost all the banks have core banking facility but sometimes passbooks are quite old and contains old account number which leads to problems in claim settlement and causes unnecessary problem to members. Therefore always get the passbook updated and submit a copy of it along with each claim form.
3. The bank account should not be a joint account.
4. Ensure that employee as well as the employer/authorised signatory has put his signature on the claim form.
5. Ensure that service particulars like Name of the member, PF Account number, date of joining, date of leaving, date of birth etc. should be correct and should not differ from the service particulars submitted earlier. If the service particulars submitted at the time of claim form differs from what was submitted earlier, then the office is constrained to return those claims for clarification. Therefore, ensure that whatever service particulars are submitted whether earlier or at the time of submission of claim form, there should be no discrepancy.
6. Along with claim form, always ensure that Form 3A (upto the period of leaving of service) is compulsorily submitted. Often due to non submission of Form 3A, claims are returned. The Form 3A, whether submitted annually or along with the claim form, should contain the Non Contributory Period details.
Pension Claim (Form -10 D) :
1. 3 (three) joint photographs (member and spouse only) should be submitted compulsorily along with the claim form. Often joint photograph of entire family including children are submitted, which is a wrong practice. Photographs of each child should be individual and separate.
2. The photographs should be attested by the employer at the back side of the photograph and not on the front side. It is a wrong practice for the employer to attest the front side of the photographs. The front side of the photographs should be signed only by APFC (Pension) who is the competent authority to authorise pension.
3. Bank account of each pensioner (even of children) in respect of one PPO (Pension Payment Order) should be separate and should be in the same bank and the same branch. Often it has been observed that separate bank account of children are not submitted and sometimes they are of different bank branches.
4. SRO Jabalpur disbursed pension only through State Bank of India or Punjab National Bank. Therefore, pension claim should be made only when the member has bank account in either of the two banks.
5. Wages as on 15/11/1995 should be mentioned in the claim form (if applicable).
6. Attested copy of date of birth certificate of children is a must. If available, the date of birth certificate of spouse should also be provided.
7. In case of death, death certificate in original has to be provided.
For Regular Pensioners :
1. Life Certificate has to be submitted in your respective bank branch every year in the month of November. If the life certificate is not received in the office by December then pension is stopped from the month of January.
Common Mistakes While Submitting Forms :
1. Form 3A not submitted along with claim forms.
2. NRS/NCP (Non Contributory Period) is not mentioned in Form 3A.
3. Photocopy of the latest passbook is not submitted.
4. Details submitted in the claim form differs from the records submitted earlier by the employer to the office.
5. Joint photographs of member and spouse is not submitted in the case of pension claims.
6. Claim form not signed by member.
7. Claim form not signed by the authorised signatory of the establishment.
8. In pension claim (Form 10D), bank details other than that of SBI and PNB are furnished. SRO Jabalpur disburses pension only through SBI and PNB.
9. In transfer claim (Form 13), both old and new PF account no. have to be mentioned correctly to avoid delay in settlement of the claim.
10. Mobile number of member/claimant are not mentioned in the claim form.
Note :
This is a common procedure to fill pension claim form all over India. But here we have described briefly only for Employees Provident Fund Organisation, Madhya Pradesh.
